Short Health Questionnaire – how do you rate yourself?

In the interest of creating something of a starting point for yourself, please take a moment to visit this survey, print it out and fill it out today.  We’ll use this as the BEFORE status.  6 weeks from now, you can look back and re-evaluate to see what progress you’ve made.  It only works though if you actually do it!

Once you’ve printed it, please also fill out the online version so that our answers can all be tallied.  All answers are completely anonymous!


Anyone is welcome to take this survey!  Please feel free to share it with your friends.  It’s a great retrospective look at lifestyle choices, and I swear it won’t hurt to fill it out!  It should only take a few minutes.
